Public School Open Enrollment, Part-Time Open Enrollment,
Early College Credit Program, and Additional Year Tuition Waivers

Collection of this data is a requirement of s. 118.51, Wisconsin Stats.

This is a claim for reimbursement of costs to transport eligible pupils to and from school who are attending school in a district other than the one in which they live in (nonresident district) under the Wisconsin full-time public school open enrollment program, an additional year tuition waiver, Part-Time Open Enrollment, or Early College Credit Program. If a pupil attends a private school, a school in the school district that they live in (resident district) or a nonresident district under a current year tuition waiver, they are not eligible for the reimbursement. Additionally, pupils must qualify for free or reduced price meals under the federal school lunch program to be eligible for the reimbursement (see information on the next page of the online form to determine if you qualify).

All claims for a family should be submitted on a single form. A claim for reimbursement may be submitted at the end of the school year. All claims must be submitted online to the Department of Public Instruction no later than July 15, 2020. Claims submitted after July 15, 2020 will not be paid.

The DPI expects to receive many more claims than it has money available. Based on prior years’ claims, it is estimated that each family’s total reimbursement will be prorated at approximately 20 percent of their full eligible amount.

Fill out all information completely and accurately. If you do not, your claim may not be paid.

Email is our primary communication tool and is a required field on this claim form. We use email to contact parents to notify them that their claim is denied, to get answers to questions about their claim, and to retrieve missing or additional information that is required for us to process their claim.
